Please find the ACC request form and guidelines below. 

The ACC Review Committee has up to 30 days to review your application. The process cannot begin until the completed application is received and you have written approval. 

To submit to the ACC Review Committee, please print out the form below and submit via email to [email protected].

The ACC Committee will send written confirmation via email to begin your project. 

Hard copies can also be mailed, but please note they do take longer to process:
